Use RentCafe to complete your Annual Recertification and submit required household, income, asset, and other supporting documentation.
Who completes: Head of household. Additional adult household members will also be required to sign certain forms.

Use this process when an HCV participant has selected a unit and the owner is requesting approval for the assisted tenancy.

Rent Increase Request (Bob.Ai)
Use this request to submit a proposed rent increase for an HCV-assisted unit. Requests are subject to applicable notice, timing, rent reasonableness, and program requirements.

This is a HUD-required form that authorizes HUD and NHA to obtain information needed to verify your household's eligibility and the amount of housing assistance you receive. By signing, you authorize required verification of income, assets, and other information used to administer your housing assistance.
Who signs: Each household member age 18 or older.

This is an NHA-required form that authorizes NHA to obtain information from third parties when needed to verify information you report about your household. By signing, you give NHA permission to request information necessary to determine or verify your eligibility and continued participation in the HCV or PBV Program.

This HUD-required form explains how information about debts owed to Public Housing Agencies (PHAs) and prior terminations of housing assistance is collected and shared. By signing, you acknowledge that you have received and understand this information and the requirements described in the form.
Who signs: Each household member age 18 or older.

This is a HUD-required declaration used to document the citizenship or eligible immigration status of each household member, including adults and minor children. Each household member must have a completed declaration as required by HUD. You should also complete a new declaration if a household member's citizenship or eligible immigration status changes.
Who signs: Each adult household member signs their own declaration. A parent or legal guardian must complete and sign the declaration for each household member under age 18.

This is an NHA-required form that explains your family’s responsibilities while receiving HCV or PBV housing assistance. By signing, you acknowledge that you have received, reviewed, and understand the program requirements and your responsibilities for maintaining your housing assistance.
Who signs: Each household member age 18 or older.

This NHA form allows you to provide a written statement or explanation when other supporting documentation is unavailable or when NHA needs additional information from you. By signing, you certify that the information provided in your statement is true and complete to the best of your knowledge.
Who signs: The household member providing the statement.

This agreement is completed after NHA approves a live-in aide as a reasonable accommodation. It explains the requirements and responsibilities that you and your approved live-in aide must follow while the live-in aide resides in your assisted unit.
Who signs: Once the need for a live-in aide has been approved, the Head of Household and approved live-in aide must sign the Live-In Aide Agreement.
Important: The household member who requires a live-in aide as a reasonable accommodation must first be approved by the Newark Housing Authority ADA Coordinator. If approval has not yet been received, complete the Reasonable Accommodation Request Form to request approval for the need for a live-in aide.

This is a HUD-required form that explains your responsibility to report complete and accurate information about your household, income, and assets. By signing, you acknowledge that you understand these reporting requirements and that providing false, incomplete, or unreported information may affect your housing assistance.

This NHA form should be completed for seasonal employment reported during Initial Eligibility, Annual Recertification, or an Interim Recertification. The form allows you to select how you would like NHA to annualize your seasonal income for purposes of calculating your housing assistance and explains your reporting responsibilities under each method.
Who signs: The household member with seasonal employment.

This HUD-required form allows the Head of Household to identify a person or organization that HUD or NHA may contact if assistance is needed regarding your housing application or tenancy. You may choose to provide a contact or indicate that you do not wish to provide one.

This HUD-required form confirms that you received information about your rights and protections under the Violence Against Women Act (VAWA). By signing, you acknowledge that you received information about the VAWA protections available to applicants and participants of HUD-assisted housing programs.

Zero Income Affidavit
This NHA-required form is completed by each adult household member who reports receiving no income from any source. By signing, you certify that you currently have no income and that the information provided is true and complete.
Who signs: Each adult household member who reports receiving zero income must complete and sign their own Zero Income Affidavit.

This NHA-required form is completed when the entire household reports receiving no income from any source. It explains how the household's living expenses, such as rent, utilities, food, transportation, and other expenses, are being paid while the household has no income.
Who signs: The Head of Household completes and signs this form on behalf of the family.
